ADMISSION GUIDELINES & CONSIDERATIONS
- Must have significant mental health concerns that cannot be successfully treated in an outpatient or less restrictive lower level of care
- Must be cognitively and emotionally capable of participating in individual, group, and family therapy
- Must be between the ages of 6 to 17
- Must be ambulatory
- Must have a minimal IQ of 70
- Must meet medical necessity requirements
- Severely developmentally impaired
- Significant health concerns that require specialized care and treatment
- Needs acute stabilization services
- Actively suicidal, homicidal, and/or exhibiting significant self-harming behaviors
- Significant Conduct Disorder diagnosis
- Need of specialized treatment (i.e., eating disorder, sexual offender, etc.)
- Mental health or behavioral diagnosis does not severely interfere with daily functioning in school, home/family, or the community
- Primary diagnosis of substance abuse
- Contact Saint Joseph’s Director of Admissions to discuss the referral
- Please email, fax, or mail the requested information to support the admission criteria (see details below for pre-admission review)
- The material is then reviewed by the referral team
- The referral source is contacted regarding the decision of the referral team.
- The application and social history are completed by the referral source and the youth’s parents or primary caregiver and are submitted to Saint Joseph’s (may fax or email and follow-up by sending originals in the mail)
- Once accepted, the Director of Admissions and the referral source work together to coordinate admission.
- Available psychiatric evaluations and/or psychological evaluations
- Information regarding prior placements, including psychiatric hospitalization
- Information regarding academic progress
- Title XIX number and Equality card (Medicaid)
- Private Insurance Card
- A legible photocopy of the front and back of the card, social security number, home address, and date of birth of the primary card holder.
