St. Joseph's Programs

St. Joseph's Children's Home/Newell Children's Center provides residential services to boys and girls ages 6-17 with primary mental health and chemical dependency problems. For a child/adolescent to qualify for admission the following conditions must be present:

  • Current problems with mental health or dysfunctional behavior that are so severe the child cannot be maintained in a less restrictive environment.
  • Child/adolescent must be ambulatory and physically and emotionally capable of participating in St. Joseph's/Newell Center residential program.

Children/adolescents may not be eligible for admission with the following conditions:

  • Acute psychiatric instability requiring 24-hour medical services.
  • Severe Mental Retardation
  • Active physical health problems requiring 24 hour medical and/or nursing care and supervision.

No child will be discriminated against because of race, sex, color, national origin, age, or handicap.

THE FOLLOWING DOCUMENTS ARE REQUIRED UPON ADMISSION:

  • Social History
  • Court Order
  • Immunization Record
  • Title XIX number and card

These forms require your signature:

  • Consent and Authorization
  • Admission Application
  • Notice of Information Practices
  • Consent for Religious Activities
  • Permission for Visitation
  • Consent for EMDR

These forms require your signature be witnessed:

  • Authorization for Release of Information for Psychiatrist
  • Professional Disclosure
  • Authorization for Release of Information for St. Joseph's Children's Home

If the Admission Application is faxed, please mail the original as soon as possible.
